Liberty Dentistry makes new patient forms simple to complete before your appointment. You can fill out your paperwork online from home, helping save time when you arrive at our Liberty office. The forms collect important details about your dental history, health background, and insurance so we can better understand your needs. Completing them ahead of time leaves more of your visit for conversation, care, and questions about your smile.

Prefer to complete your forms in person? You are welcome to do that, too. Arrive about fifteen minutes before your appointment, and bring a valid photo ID and insurance card if applicable. We’d love to help if you have questions along the way.

Dr. Moses earned his dental degree from the University of Oklahoma and practiced in South Carolina before returning to North Carolina to be near family. He gives patients the information they need to make confident decisions and enjoys putting new technology to work for their care.

Dr. Jonathan Breazeale

DMD

Dr. Jonathan Breazeale earned his DMD from Tufts University School of Dental Medicine and provides general and restorative care. He believes great dentistry starts with listening to each patient’s goals. A member of the ADA and North Carolina Dental Society, he enjoys golf, tennis, and cheering on the Hurricanes.

Fantastic dental office! The staff are all very friendly and knowledgeable, Trisha does fantastic work on the cleanings and Brittnie has been a rock star with my aligners.
